Myofascial Release
Some of you may have come across the terms “fascia” or “myofascia” — Perhaps it’s been mentioned by a healthcare professional, or something you’ve looked up after struggling with persistent pain that doesn’t seem to have an obvious explanation. Let me try to explain what fascia actually is, what role it plays in the body, and how a technique called myofascial release may be able to help.
First, Let’s Break Down the Terminology
In Latin, myo translates to muscle and fascia means band. Put them together and myofascia refers to the relationship between muscles and the remarkable connective tissue that surrounds them.
Myofascia is a continuous, three-dimensional web of dynamically adapting, fluid-filled connective tissue that supports, encompasses and protects every structure in the body — including muscles, tendons, ligaments, joint capsules, nerves, organs and bones.
It has been described as the largest system in the body and possesses ten times more sensory nerve endings than muscle tissue. Not only does it respond to the physical demands placed upon it through postural adaptations and mechanical load, but it also aligns beautifully with the central osteopathic principle that no anatomical structure works in isolation.
What Are Fascial Restrictions and Why Do They Matter?
Many people experience pain due to what are known as fascial restrictions. These restrictions can be caused by a range of physical and emotional events — including trauma, surgery, poor posture and inflammation — and can cause pain and discomfort not only at the site of restriction, but also in more distant parts of the body.
This phenomenon is often referred to as myofascial pain syndrome, characterised by local pain and tenderness in specific areas of muscle known as trigger points.
One of the most important things to understand about fascial restrictions is that they do not show up on standard imaging such as X-rays, MRI or CT scans. This is one of the reasons why the underlying cause of a person’s symptoms can sometimes be missed or misdiagnosed — and why many people spend months or even years searching for answers before finding relief.
In response to injury or stress, fascia alters its natural structure, causing the body to lose its physiological capacity to adapt. I sometimes describe it as putting on a jumper over a shirt where the shirt sleeves get twisted inside. It takes time and patience to work the sleeves back into alignment — but once you do, you immediately notice the freedom of movement that was previously lost.
If left unaddressed, these fascial deformations can create tensile forces on pain-sensitive structures of up to 2,000 pounds per square inch. Is it any wonder fascia can be the source of so much persistent and wide-ranging pain?
What Is Myofascial Release?
Myofascial release is a hands-on, non-invasive, individualised osteopathic technique that addresses areas of tissue restriction and adhesion that may be contributing to a person’s pain.
What’s particularly interesting about this approach is how fascia responds to touch — specifically, the type of sustained, gentle pressure that osteopaths are trained to apply. In fact, it appears that the slower the applied pressure, the greater the release. This may seem counterintuitive when compared to the short, sharp techniques that produce the familiar “crack” or “pop” associated with osteopathic manipulation — but fascia operates by different rules.
Myofascial release involves a series of very gentle, sustained techniques primarily aimed at releasing tension and adhesions within the fascia. These techniques can sometimes cause what’s known as a “red flare” — or vasodilation — which is simply a result of increased blood circulation in response to the release of tissue restrictions. It’s a completely normal and often encouraging sign that the tissue is responding.
What Are the Benefits of Myofascial Release?
The benefits are extensive, and in my experience, often go beyond what patients initially expect.
Pain relief
One of the most significant benefits is its ability to alleviate chronic pain associated with conditions such as back pain, fibromyalgia, headaches and sports injuries, as well as improving sleep and quality of life. These aren’t just clinical observations — they’re supported by peer-reviewed research, which I’ve linked to directly above.
Improved mobility
Many patients notice they feel significantly less “stiff” following treatment, allowing them to return to activities they enjoy without the discomfort that had been holding them back. This applies whether you’re a runner dealing with a persistent hip or groin issue, a desk worker with neck and shoulder tension, or an older adult managing joint stiffness.
Improved circulation
By releasing tissue restrictions, myofascial release aids healing by delivering essential nutrients to the tissues while helping to remove the metabolic waste products that can contribute to ongoing pain and inflammation.
Stress and tension relief
Fascia responds to both physical and emotional stress. Myofascial release can help reduce levels of physical tension in the body, making it an excellent option for those managing stress and anxiety alongside musculoskeletal symptoms.
A genuinely holistic approach
What I particularly value about myofascial release is its simplicity. No needles, no massage guns, no shockwave machines. Just skilled, sustained, gentle hands-on contact — applied with intention and patience. It can also be used as part of an ongoing maintenance programme for general wellbeing, not just for acute or chronic pain.
Who Might Benefit from Myofascial Release?
Myofascial release can be a valuable part of treatment for a wide range of presentations, including:
- Persistent back pain or neck pain that hasn’t fully responded to other treatment
- Headaches with associated neck and upper back tension
- Shoulder, hip or knee stiffness and restricted movement
- Sports injuries and overuse conditions
- Work-related musculoskeletal strain
- Fibromyalgia and widespread pain conditions
- Those who prefer a very gentle treatment approach
- Patients who are nervous about hands-on treatment or manipulation
Is Myofascial Release the Same as Massage?
Not quite. While both involve hands-on contact, the two techniques differ significantly in their approach and intention.
Massage typically involves rhythmic pressure and movement applied across the surface of the body. Myofascial release involves sustained, still pressure held at the point of fascial restriction — waiting for the tissue to soften and release rather than working against it. The pace is slower, the pressure is lighter and the focus is on listening to the tissue rather than working on it.
What to Expect During a Myofascial Release Session
As with all treatment at Chelmsford Osteopathy Clinic, a session begins with a thorough assessment of your symptoms, history and goals. Myofascial release may be used as the primary technique or alongside other osteopathic approaches — depending on what’s most appropriate for you.
The technique itself tends to feel gentle and sometimes deeply relaxing. Some people notice changes during the session itself; others experience gradual improvement over the following days. As with most forms of treatment, some mild post-treatment soreness can occasionally occur — similar to the feeling after a deep stretch.
